Hang a curtain too close to a fireplace and you run the risk of setting your house ablaze. Drive a car on a pitch-black night without headlights, and you dramatically increase the odds of smacking into a tree.

These are matters of common sense, applied to simple questions of cause and effect. But what happens, asks systems-behavior expert Charles Perrow, when common sense runs up against the complex systems, electrical and mechanical, with which we have surrounded ourselves? Plenty of mayhem can ensue, he replies. The Chernobyl nuclear accident, to name one recent disaster, was partially brought about by the failure of a safety system that was being brought on line, a failure that touched off an unforeseeable and irreversible chain of disruptions; the less severe but still frightening accident at Three Mile Island, similarly, came about as the result of small errors that, taken by themselves, were insignificant, but that snowballed to near-catastrophic result.

Only through such failures, Perrow suggests, can designers improve the safety of complex systems. But, he adds, those improvements may introduce new opportunities for disaster. Looking at an array of real and potential technological mishaps--including the Bhopal chemical-plant accident of 1984, the Challenger explosion of 1986, and the possible disruptions of Y2K and genetic engineering--Perrow concludes that as our technologies become more complex, the odds of tragic results increase. Book Description

Normal Accidents analyzes the social side of technological risk. Charles Perrow argues that the conventional engineering approach to ensuring safety--building in more warnings and safeguards--fails because systems complexity makes failures inevitable. He asserts that typical precautions, by adding to complexity, may help create new categories of accidents. (At Chernobyl, tests of a new safety system helped produce the meltdown and subsequent fire.) By recognizing two dimensions of risk--complex versus linear interactions, and tight versus loose coupling--this book provides a powerful framework for analyzing risks and the organizations that insist we run them.

The first edition fulfilled one reviewer's prediction that it "may mark the beginning of accident research." In the new afterword to this edition Perrow reviews the extensive work on the major accidents of the last fifteen years, including Bhopal, Chernobyl, and the Challenger disaster. The new postscript probes what the author considers to be the "quintessential 'Normal Accident'" of our time: the Y2K computer problem.

This book will change the way you think. It's as simple as that. Really eye-opening and persuasive arguments and examples as to why the absence of accidents per se is not an adequate indicator of a system's safety. Near-accidents are a better metric. Fascinating!

4 out of 5 stars Recommend but there are some errors in the details.......2006-04-26

I purchased and read the book for the concepts but enjoyed the examples. However, I did find several errors in the events in which I am familiar. Specifically those of space flight. In as little as four pages (pp267-270) he makes three errors. It makes me wonder about the accuracy of the rest of the book's examples.

Just for the record:
1) He attributes the second orbital flight of Mercury to Scott Crossfield when in fact it was Scott Carpenter. Crossfield was an X-15 test pilot of great skill, while Carpenter has been noted as the "worst astronaut in the program" by Chris Kraft. There are many accounts of Mr. Kraft openly wondering how Carpenter got to be an astronaut in the first place let alone being allowed to fly into space. This might better explain the events instead of it being a process - system problem.

2) It was almost certain then, and now after the recent recovery of Gus Grissom's Mercury capsule absolutely confirmed, that Gus did NOT blow the hatch. This would have been know at the time of my copy's printing.

3) It was Glenn's heat shield not the landing pack, which the status light indicated had come loose. This would have explained the reluctance of ground control to inform the astronaut since not much could have been done if true.

Of course these are minor and don't lead the author to any significantly different conclusions than if they were corrected. But the sloppiness does make me lower it to a 4 instead of 5 stars.

having obtained a second hand copy of "Normal Accidents" in the original 1984 edition, it is fascinating to see how many of the accident scenarios are still valid today. When compared with Lagadec's thesis published in 1982 ("Major Technological Risk", Pergamon Collection Futuribles, o. o. p.), the style is easier to understand, and the book is leaning a little more towards US culture and thinking of the 1980s. Incidentally, Lagadec himself reflected in a 1997 paper how "Normal Accident Theory" had become one of the generic views that engineers and/or sociologists had developed since the early 1990s. Even in 1996, Hood and Jones ("Contemporary Debates in Risk Management") quote Perrow as one of the significant contributors to the fundamental debate in risk management.
I very much enjoyed reading one of the early works in disaster and accident theory. Recommended without hesitation, and essential when you are looking for a deeper understanding of the theories that are around today.

Perrow sets out a framework for thinking about the causes of 'normal' i.e. unpreventable accidents by looking at the coupling of features in a system and then overlays the potential severity of a failure to draw conclusions about the real reasons for catastrophes beyond the often useless explanations of user error and acts of God. This is a book that is more often quoted than read, which is a pity as it is well written and thoughtful, though the final attacks on social power relations are a bit over the top.

This book is well written and informative. Gets sort of heady at times with the analysis but well worth the read if you enjoy system accident analysis.

        I purchased this book on the premise that Michael Mandel, being a senior business editor at Business Week, would have phenomenal insights about the growing uncertainty in our society. He presents a beautiful synopsis with excellent facts and figures. He summarizes that regardless of what an individual experiences, as an "idiosyncratic risk" group, or a "structural risk" group (meaning selective termination notices or termination based on complete industry shut-down), the American economy will continue to grow.

        Such factors include government policy, international trade, corporate restructuring, overseas manufacturing, improved education, entrepreneurs and other measurements such as health and insurance products. According to Mandel, the objective is to minimize risk in our society by improving our economic prospects, as a nation, in the global economy.

        Unfortunately, this is where brilliance 'tumbles off the cliff.' As expected (determined from the deteriorating progression of the chapters), several of his examples are fatally flawed. Several of his forecasts are fundamentally in error along with inaccurate facts and figures, and I found myself in denial, desperately searching for his expert recommendations. I was even willing to accept a few cheap answers to all his carefully illustrated points!

        For example, Mandel raves about how spectacular salaries in the future are tied to individuals choosing aspiring technology careers, yet he admits this career path is extremely unstable. He also states that the Wal-Marts and other 'mature' industries are stable but low-paying (relatively speaking to the following examples). First, since when does McDonalds, and now Boeing, have time and resources to be complacent and provide stability to its work force? Didn't Wal-Mart challenge the complacent industries? What about Airbus and the debate about subsidies for both aerospace firms? Which of the two 'risktakers' (high-paid unstable v low-paid stable) possess the better credit rating? Which group can afford the mortgage on the new house, the car payments, and the new computers?

        Furthermore, what happens if a student, a 'risktaker' based on his conditions and tolerance of risk, obtains a significant amount of training and the industry disappears or the network established turns out not to guarantee employment because he or she went to a second-tier or third-tier school? Let's be honest about who gets anointed to the best jobs, regardless of talent! In other words, there is nothing stable in this economy. What if? What if? Cheap shot!

        Also, Mandel's treatment of total quality management is alarming. He says "the problem is that the transferred responsibility under a TQM system increases pressure on workers and managers." Yeah, so where's the problem? That is a POSITIVE manifestation for improving accountability and instilling pride on the individual in the workplace!

        I hope Mandel writes another book to wrap up PART 1, The High Risk Society, because I am convinced it will sell extremely well, and finally, he can provide us with his expert recommendations.

        5 out of 5 stars Terrific Book -- Insightful and Prophetic.......2002-01-04

        This guy really knows his stuff. The changing nature of business risks is something of value to every manager, regardless of level. I was really impressed with his predictions, especially that of the domestic steel industry collapse based on international markets.

        The book discusses a variety of themes involving how today's economy requires that companies embrace risk to chase the rewards people want.

                  Yes, we can all agree that mainstream history is nearly 100% BS due to politics, economics, ego, problems with dating techniques, and various conspiracies. Agreed. But, I've been researching the distinct possibility that human history (in terms of civilizations) are much more ancient than we've been told, so coming across this book was very interesting to me. I wondered how Fomenko could be wrong (if at all) because he is very persuasive in his presentations. Then it dawned on me. If at previous times in prehistory, due to the various catastrophies that are well documented (comets, asteroids, planetary disruptions, plasma discharge, pole reversals, etc) the Earth was in a different position in relation to the sun, different tilt on its axis, different orbit, different rotation (in terms of velocity and DIRECTION), and the continents were in different positions, then would this not cause the ancients to see the sky (constellations) differently? In other words, is Fomenko making erronious assumptions about the physics of the Earth in pre-history, which then corrupt his data with regards to dating the relevant astrology? The last event to seriously disrupt our planet occured roughly 3500 years ago, according to other good researchers, so is it possible Fomenko has been confused by this? The vastly different physics of our planet in the not so distant past may explain this confusion, which is not to say the "mainstream" version of history is correct; on the contrary. I am not an expert in these fields, but wanted to see if this idea could spark discussion.

                  Will people ever read before spamming? Yes, Jesuits could not rewrite world history alone, they had help. Anyway, Dr Prof Acad A.Fomenko does not point to jesuits as the driving force of world wide history manipulation in published volumes 1,2,3;, actually he barely mentions the poor devils. Check it with 'Search inside' feature, please. China is rarely mentioned either, in fact, Dr Fomenko is completely eurocentric. Right, his theory contradicts all mainstream schools of history, because in their actual state they are all built on blatantly erroneus chronology. You don't need a mysterious cabal (conspiracy) to falsify history, the falsification is its modus operandi. It is inherent to history(ians) to falsify (distort) events, as it is inherent to humans to boast as it is inherent to power (authority) to legimize itself by referrring to glorious past made to its own order. Dr Prof Fomenko and team have identified scores of instances of such manipulation in Russian, European, etc.. history, and delivered valid statistical proof thereof. His own 'reconstruction' is completely another story. Forget c14 as a valid method of dating. W.Libby has initially discovered a brilliant method of INDEPENDENT dating. Too bad, c14 method has become a joke after a forced marrige with dendrochronology with consensual chronological scale inbuilt. Radiocarbon method can't stand blind tests, but is so very productive as a rubberstamp.

                  5 out of 5 stars Accepted History & Chronology Must Be Changed. .......2007-04-09

                  There is no doubt that history as most know it is a sham, & institution's version of History both University & Church is fradulent & inaccurate. Everything was established with an agenda, The real "Dark Ages" are now when we have access to incredible amounts of information past authorities & more important 'common folk' didn't have but our institutions & educators are slow to evolve because of what has ignorantly & arrogantly been taught for too long. This is on many subjects not just Chronology.

                  For anyone to question "Why would a Mathematician have anything credible to say of History?" The answer is from Dr. Fomenko's preface in the book: "It would be worthwhile to remind the reader that in the XVI-XVII century Chronology was considered to be a subdivision of Mathematics." These volumes could possibly be some of the most important works to date & should be read by everyone with an interest in History, especially professors & educators who have a duty to the public. I have read both books & must say that 'Chronology 1' has some very eye opening & revolutionary information. Even if these volumes are part true the implications are profound & opens the doors to further investigations & questions which must be done. I speak several different lanquages & must say the logic Dr. Fomenko uses with "inflection" of words & words being read from left to right in one region & right to left in another then written backwards, the removal of vowels & get down to basics of words, or different cities & locations having the same name etc. is correct. Vowel usage has always been optional & varied, actually complicating linquistics & study. The first thing one has to understand is that words never had a fixed spelling in history like we do now, the spelling of words was mutable & regional, as well as names & titles of people were vast, varied & changed, NOTHING WAS FIXED or understood linear. Matters of Life & Death as well as financial profiteering yesterday & today were & are made with ignorant, illogical & conspiratorial views of history & reality, it's time people get closer to the Truth & society collectively grow up.

                  4 out of 5 stars History as Science Fiction.......2007-01-10

                  Anatoly Fomenko has written a very intriguing book, full of pictures, charts, and computer 'proof' of his thesis: backwards of AD900 we don't really know what happened or when. Between AD900 and AD1600 there is more certainty, but there is still a lot of fuzzy ground, and things don't get reliable until we get past the 1600's where the printing press made it very difficult for the perpetrators of this timeline manipulation to change anything that had been committed to print. The Dark Ages did not happen. Books were burned for a reason. One organization has doubled the actual length of its existence by expanding the real chronology. Read why.

                  I had always wondered why Christ died about AD33 and yet men waited until the 11th century to form the Knights Templar, the Cathars, etc and go after the Holy Land by force. Why the 1000 year gap? Turns out there wasn't more than a 10-12 year gap and he proves it using astronomy. This also implies that the planet is not as old as we have been told, and current Christian and other creationist scientists are already championing that idea without being aware of Fomenko's book. The two groups, creationist scientists and the Russian mathematical analysts corroborate each other. Fascinating.

                  Of course, all this flies in the face of what we have been told traditionally is the 'proper' chronology of western civilization, and most readers will experience 'cognitive dissonance' in reading this book. It means that our history going backwards from AD1600 becomes progressively more incorrect and unreliable until it cannot be trusted at all... in the space of 700-800 years.

                  Naturally, the curious, open-minded reader will want to know WHO did this, WHY, and did any of the events we think of as really ancient ever happen?
                  Dr. Fomenko is a respected scientist/mathematician at Moscow State University who has already answered these questions to the satisfaction of his initially skeptical colleagues. Most of them are now believers, a few still refuse to believe (the usual diehards), and of course the western press has ignored Fomenko's work -- for obvious reasons when you read the book. The ones who perpetrated this chronology ruse have a lot to answer for. They are still with us. That's why this book is a well-kept secret.

                  I gave the book a 4-star rating because I was unable to check out some of his claims; those I checked were as he said. But if even 1/3 of his claims are true, this punches a big hole in what we think is our history, the meaning of western civilization, our educational process (for repeating the ruse as gospel), and the trustworthiness of the organization that perpetrated this ruse, well-intentioned or not.

                  This book relates to current research into a Young Earth paradigm, to John Keel's discoveries about our planet, and Fr Malachi Martin's insights (in his now out-of-print books). We are indeed sheep who are manipulated and kept ignorant -- for a reason. While knowing what these men have to say may be the "booby prize" (as in: 'what can you do with this knowledge?'), it will provide interesting reading. Didn't someone say: "...and the Truth will set you free."?? For you to judge if this book contains the truth.

                  This book is not for the person just getting an intiation into school law. It is too condense and only gives blurbs of the verdicts in each case by quoting the appellate and US supreme court judges which are oft times convoluted, the author leaves out an indepth explanation of the nuts and bolts. I refer to this book as the Reader's Digest version of school law. Also, the print is microscopic which is really hard on the eyes.
