Clinical Applications of TGF-ß Chairman: M.B. Sporn 1991 TGF-ß, originally identified as a transforming growth factor, is now known to exist as a family of factors with similar properties. These factors promote cell proliferation in some tissue types or conditions and induce cell differentiation in others. This book contains the latest information on the molecular biology of this family and the cognate receptors. TGF-ß regulates the expression of components of the extracellular matrix, and thus cell adhesion and migrationprocesses that are particularly important during development and tumorigenesis. TGF-ß has an effect on many cell types, which means that it, or its antagonists, may have clinical applications in diseases of many different tissues. It is already showing promise as a therapeutic agent in bone and wound healing. Increased expression of TGF-ß is associated with the fibrosis that occurs in chronic liver disease (TGF-ß1) and in pulmonary fibrosis of the lung (TGF-ß1 and 2). Treatment with an anti-TGF-ß1 antiserum attenuated the histological manifestations of disease in an experimental model of glomerulonephritis. In the haemopoietic system, TGF-ß arrests the growth of the most primitive stem cells and could therefore be used as a bone marrow protrctive agent during chemotherapy. In contrast, TGF-ß2 is produced by glioblastomas and may contribute to the malignancy of these tumours by impairing tumour surveillanceanother case in which treatment with an antagonist may be useful. The complex effects of TGF-ß in different tissues make it a very exciting agent, but one which must be used with care. This book evaluates the behaviour of the TGF-ß family in many contexts and provides a basis for future clinical application. There is widespread public concern about hazardous chemicals that are contained in air, soil, water and food. Policy has therefore adopted a series of laws and regulations concerning emissions into and concentration levels in different media including food. As policy makers do not only have to consider the protection of the environment but also need to ensure a well-functioning economy at the same time, these limit or target values need to be set in a balanced way. The main problem, however, is to compare the costs for achieving these targets with the benefits to society by having a smaller exposure to hazardous substances (cost-benefit analysis).
This book sets out to improve the reliability of cost-benefit analyses particularly of hazardous substances present in air, water, soil and food. It suggests that the human health risk assessment of chemicals is performed in a bottom-up analysis, i.e., following a spatially resolved multimedia modelling approach. In order to support cost-benefit analyses, the approach is accompanied by monetary valuation of human health impacts, yielding so-called external costs. Results for selected priority metals show that these external costs are small compared to those by the classical air pollutants and involve rather long time horizons touching on the aspect of intergenerational equity within sustainable development. When including further hazardous substances, the total external costs attributable to contaminants are expected to be more substantial.
